Incorrect page size. We know that sounds like a simple thing to stuff up, but this is the most common mistake we see. Most often it is because the designer only started designing the card, brochure or letterhead with the default page size of the application, which may be letter or A4. 

This is a problem, when supplied to a printer, and often have automatic "imposition" or design applications for their work in a "press sheet. When the page size is wrong that means it is not the correct position and can cause it to break in the wrong position. Obviously, you do not want this!
